Patients:
Please be prepared to give the following information
if your physician has you call to schedule your test:
- Name, Social
Security Number, Date of Birth, exam and diagnosis, name of
physician ordering the test and the type of insurance you have.
- Our staff
will remind you to bring any necessary paperwork with you, ask
whether or not you have been given the necessary information
about preparing for the exam and then will schedule a time that
is convenient for you.
- If you
need to reschedule your exam, you can call us at the same number
and we will be happy to change the date of your exam.
- If your
physician has scheduled the exam, but you have questions, please
feel free to call us and we will try to answer any questions
you may have.
- If your
physician has any films that he or she would like you to bring
with you to your exam, please remember to do so.

For
Physicians and Office Staff:
- Please
have the patient's chart in front of you when scheduling, and
know the patient's diagnosis.
- If the
patient is going to schedule the appointment time themselves,
we will still need a verbal order from you. This is to ensure
we have the correct exam ordered and all the information needed
to do the exam.
- Walk-in's
for plain film x-ray are welcome, however we will still need
you to phone us with a verbal order, send the script with the
patient or fax the script to us at 419-841-2711.
- It is important
and helpful for us to know if any previous studies (of the same
anatomical area) were done. If previous studies were not done
at our location we ask that you fax us a copy of the previous
report for comparison. We will obtain previous films.
- Please
review the exam preparations with the patient. If you don't
have time or you are not sure how they should prepare, please
have them call our office.
- We will
give a reminder call to each patient the day before their exam.
We will let you know if the patient does not show up for their
scheduled exam or, if for any reason, they were unable to complete
the exam.
- While we
will be happy to assist in any way with insurance questions
or other concerns,
it is still the patient's responsibility to know what exams
their insurance will cover and
where their insurance allows them to have the exams done.
